The form below collects basic information about the applying organization/entity to determine the potential for community partnership. To be considered, the applying community partner address must be within Washington State and must be a verifiable entity.

Applications are reviewed based on various factors including:

• Mission/Vision alignment

• Commitment to serve the Black community

• Demonstrated ability to provide culturally concordant care and/or programming, and directly engage with, and seek feedback

• Ability to collect and submit accurate recipient data, place orders online, and pick up directly from BLBKRY or have a physical address to receive deliveries.
